Is DVP or AFI better for fetal surveillance?

DVP (single deepest pocket ≥2 cm) has lower false-positive rate for oligohydramnios than AFI <5 cm without improving detection of adverse perinatal outcomes (COCHRANE 2019). DVP is now preferred in many centers.

What causes oligohydramnios?

Uteroplacental insufficiency (FGR), post-dates pregnancy, PPROM, obstructive uropathy, renal agenesis. AFI <5 cm at term increases risk of non-reassuring fetal status in labor, meconium aspiration, and cord compression.

What causes polyhydramnios?

Idiopathic (60-70%), fetal GI anomalies (esophageal atresia, duodenal atresia), CNS anomalies (impaired swallowing), maternal diabetes, fetal anemia (Rh disease), twin-twin transfusion syndrome.

Amniotic Fluid Index (AFI) and Deepest Vertical Pocket Threshold

Calculate AFI (sum of four-quadrant deepest pockets) and deepest vertical pocket (DVP). AFI <5 cm = oligohydramnios; >24 cm = polyhydramnios. DVP <2 cm = oligohydramnios; >8 cm = polyhydramnios.
