BASDAI ≥4 (out of 10) indicates high disease activity and is often used as a threshold for biologic therapy consideration, though current guidelines prefer ASDAS.

How is BASDAI calculated?

Six NRS 0-10 items. Questions 5 and 6 (morning stiffness duration and severity) are averaged before summing. Total = (sum of items 1-4 + average of items 5-6) ÷ 5.

Is BASDAI still used in clinical practice?

Yes. While ASDAS has largely replaced BASDAI for biologic therapy decisions in guidelines, BASDAI remains widely used clinically for patient communication and historical comparison.
