What are the BPS subscales and scoring ranges?

BPS has three subscales each scored 1–4: Facial expression (1 = relaxed; 2 = partially tightened; 3 = fully tightened; 4 = grimacing). Upper limbs (1 = no movement; 2 = partially bent; 3 = fully bent with finger flexion; 4 = permanently retracted). Compliance with ventilation (1 = tolerating; 2 = coughing but tolerating most of the time; 3 = fighting ventilator; 4 = unable to control). Total score 3 (no pain) to 12 (maximum pain). A score ≥6 suggests significant pain requiring intervention.

Is BPS valid in all ICU patient populations?

BPS is validated for mechanically ventilated, sedated, non-communicative adult ICU patients. It is less appropriate for patients on high-dose neuromuscular blockers (facial and limb movements are absent, leaving only ventilator compliance as a pain signal). For spontaneously breathing or non-ventilated patients, the CPOT (which includes vocalization as a component) may be preferred. BPS is not validated for pediatric ICU populations, where the FLACC scale is commonly used.
