Is there disease-modifying treatment for CADASIL?

No proven disease-modifying therapy exists. Management focuses on vascular risk factor control, antiplatelet therapy for secondary stroke prevention (avoiding anticoagulants unless AF is present), and cognitive rehabilitation.
