How is HSP distinguished from primary lateral sclerosis?

HSP is distinguished by family history, genetic testing, normal cognition, and absence of upper limb or bulbar involvement in pure forms. PLS has sporadic onset, slower progression, and may affect upper limbs. Genetic panel testing is essential.
