What is periacetabular osteotomy (PAO)?

PAO (Bernese PAO, Ganz technique) redirects the acetabulum to improve femoral head coverage in dysplasia. Indicated for LCEA <20-25 with symptoms, age <40, and preserved joint space. 10-year survivorship 90% for moderate dysplasia.

How does dysplasia differ from FAI?

Dysplasia (LCEA <20): insufficient acetabular coverage causing subluxation stress on labrum and cartilage. FAI (cam/pincer): excessive bone causing impingement. Both can coexist (combined deformity). Distinguishing the primary pathology guides surgical approach.

Wiberg Center-Edge Angle and Hip Dysplasia Severity

Calculate lateral center-edge angle (LCEA, Wiberg angle) and anterior center-edge angle (ACEA) from hip radiographs to grade acetabular dysplasia. LCEA <20 degrees = dysplasia, 20-25 = borderline, >25 = normal.
