Grade 1: asymptomatic or mild headache. Grade 2: moderate-severe headache, neck stiffness, no neurological deficit. Grade 3: drowsiness or confusion, mild focal deficit. Grade 4: stupor, hemiparesis. Grade 5: coma, decerebrate posturing.

How does Hunt-Hess grade influence aneurysm treatment timing?

Grades 1–3 are treated with early (within 24–72h) coiling or clipping. Grades 4–5 carry high surgical risk; treatment may be deferred until clinical improvement. Poor-grade SAH has 60–90 day mortality of 40–60%.
