CAP = age × (CAG − 35.55). Scores >100–120 indicate highest exposure and are used to enrich prevention trials. CAG repeats 36–39 have incomplete penetrance; ≥40 are fully penetrant.

Can CAG repeat length predict onset age in Huntington disease?

CAG length explains ~60–70% of variance in age at motor onset. CAG 42 predicts mean onset ~37 years; CAG 50 predicts ~27 years. The CAP score adds current age for dynamic risk estimation.
