Normal lyso-Gb1 is <7.8 ng/mL. Untreated Gaucher type 1 typically ranges 50–2000 ng/mL. On ERT, levels typically fall 70–90% within 6–12 months. Target levels on treatment: <10 ng/mL.

Why is lyso-Gb1 preferred over chitotriosidase in Gaucher monitoring?

Approximately 6–8% of patients are homozygous for the chitotriosidase pseudodeficiency allele, making it unmeasurable. Lyso-Gb1 is unaffected by this polymorphism and directly reflects substrate accumulation.
