CR after one course of 7+3: favorable risk approximately 80–85%, intermediate 65–70%, adverse 40–50%. Secondary AML and therapy-related AML have CR rates of 30–40% with 7+3 alone; CPX-351 (liposomal daunorubicin-cytarabine) improves outcomes in these subgroups.

Is daunorubicin 90 mg/m2 superior to 60 mg/m2 in 7+3?

The ECOG-ACRIN E1900 trial showed that daunorubicin 90 mg/m2 improved CR rate and OS in patients under 60 years. For patients 60 or above, 90 mg/m2 did not improve outcomes and increased toxicity. Idarubicin 12 mg/m2 x3 days is an alternative to daunorubicin 60 mg/m2.
