What WBC defines high-risk APL?

WBC above 10,000/mm3 at diagnosis defines high-risk APL with increased differentiation syndrome (DS) risk. DS can occur in low-risk APL too (15–25% of cases). Prophylactic dexamethasone 10 mg IV BID starting on day 1 of ATRA is recommended for all APL regardless of WBC.

What are the features of APL differentiation syndrome?

DS criteria: 3 or more of: fever, dyspnea, weight gain above 5 kg, pulmonary infiltrates, pleural or pericardial effusion, hypotension, or renal failure. Mild-moderate DS: continue ATRA with dexamethasone. Severe DS: hold ATRA, add arsenic trioxide if available.

Assess acute promyelocytic leukemia differentiation syndrome risk based on presenting white blood cell count, with prophylactic dexamethasone protocol for high-risk presentations (WBC above 10K).
