Why are BCAAs important in hepatic encephalopathy?

In cirrhosis, plasma BCAA (valine, leucine, isoleucine) levels are low while aromatic amino acids (AAA: phenylalanine, tyrosine) are elevated. The Fischer ratio (BCAA:AAA) is normally > 3; in cirrhosis it falls to < 1. Elevated AAAs compete with BCAAs for blood-brain barrier transport, potentially contributing to HE.

What is the recommended BCAA supplementation dose?

Oral BCAA supplementation: 0.25 g/kg/day as per EASL guidelines. Commercial formulas (HepaticAid, Aminoleban): 3-4 packets/day. Maintain protein intake 1.2-1.5 g/kg/day total (BCAAs + standard protein); protein restriction worsens HE and sarcopenia.

Does BCAA supplementation reduce HE episodes?

Meta-analyses show BCAA supplementation reduces overt HE episodes and improves nutritional status in cirrhosis. Effect on survival is less clear. Recommended as adjunct in patients with frequent HE despite lactulose/rifaximin.
