CVID requires: IgG markedly reduced (>2 SD below normal for age, typically <500 mg/dL), IgA and/or IgM reduced, poor vaccine responses to at least 2 polysaccharide antigens, onset after age 4, exclusion of other causes. Male:female ratio approximately 1:1, unlike XLA.

What IVIG dose is used in CVID?

Starting dose: IVIG 400–600 mg/kg every 3–4 weeks. Titrate to maintain trough IgG >500 mg/dL, with higher targets (700–800 mg/dL) for patients with recurrent pneumonias or structural lung disease. Subcutaneous IgG (SCIG) is an alternative with more stable levels.
