At what cyclophosphamide dose is mesna required?

Mesna is recommended for doses above 1 g/m2/day. Standard mesna dose is 60–80% of the cyclophosphamide dose, divided and administered at 0, 4, and 8 hours. Continuous IV mesna infusion is used for high-dose protocols above 2–3 g/m2.

Can mesna prevent all cases of hemorrhagic cystitis?

Mesna significantly reduces but does not eliminate HC risk. Aggressive IV hydration (3 L/m2/day), frequent voiding, and urine dipstick monitoring are essential adjuncts. Late hemorrhagic cystitis (months to years post-therapy) is not prevented by mesna.
