Approximately 30–40% of EGPA patients are ANCA positive, usually MPO-ANCA (p-ANCA). ANCA+ EGPA has higher rates of glomerulonephritis, mononeuritis multiplex, and alveolar hemorrhage. ANCA- EGPA tends to have more cardiac and eosinophil-driven tissue damage.

How is EGPA distinguished from hypereosinophilic syndrome?

EGPA requires asthma, eosinophilia >1500/µL, and at least one of: sinonasal involvement, neuropathy, pulmonary infiltrates, cardiomyopathy, or glomerulonephritis in vasculitic context. HES lacks the vasculitic features and granulomas.
