ARDS (ARDSnet): PaO2 55 to 80 mmHg (SpO2 88 to 95%). General ICU permissive target: PaO2 >60 mmHg (SpO2 >92%). Conservative oxygen therapy (ICU-ROX, LOCO2): SpO2 88 to 95% does not increase mortality vs liberal oxygenation and may reduce ICU LOS. Hyperoxia (PaO2 >100 mmHg) associated with increased mortality in post-cardiac arrest and ARDS patients.

How does increasing FiO2 affect PaO2 in ARDS vs normal lungs?

Normal lung: increasing FiO2 by 0.1 increases PaO2 by approximately 50 to 60 mmHg. ARDS: poor V/Q matching and shunt fraction mean FiO2 changes have proportionally less effect. True intrapulmonary shunt (>30%): FiO2 increase ineffective regardless. PEEP is more effective than FiO2 for ARDS oxygenation by recruiting collapsed alveoli and reducing shunt fraction.
