Anti-Xa is preferred in: elevated baseline aPTT (antiphospholipid syndrome, lupus anticoagulant, factor VIII elevation), heparin resistance (anti-Xa therapeutic but aPTT subtherapeutic), obesity (aPTT less reliable), and when aPTT reagent has poor heparin sensitivity. Target UFH anti-Xa 0.3–0.7 IU/mL for therapeutic anticoagulation.

How is heparin dosing adjusted based on anti-Xa?

Nomogram adjustments based on anti-Xa: <0.1 IU/mL: bolus 80 U/kg + increase rate 4 U/kg/h; 0.1–0.2: bolus 40 U/kg + increase 2 U/kg/h; 0.3–0.7 (therapeutic): no change; 0.71–0.99: decrease 1 U/kg/h; ≥1.0: hold 60 min, decrease 3 U/kg/h. Recheck anti-Xa 6 hours after any change.
