When should pericardiocentesis be performed?

Pericardiocentesis is indicated for cardiac tamponade (hemodynamic compromise), large effusion (>20mm) with suspected bacterial/TB/malignant etiology, or large symptomatic effusion not responding to NSAIDs/colchicine.

How is pericardial effusion size graded?

Pericardial effusion: small 20mm. Tamponade physiology (RA/RV diastolic collapse, respiratory variation >25%) can occur even with moderate effusions.

What is the ESC recommendation for drainage timing?

ESC pericardial disease guidelines recommend: urgent drainage for tamponade (Class I), drainage for large effusions (>20mm) after 3 months of conservative therapy (Class IIa). Subxiphoid approach preferred.
