Lithium clearance · 25% of CrCl. Dose guidance: CrCl >90 mL/min: 1200–1800 mg/day; CrCl 60–89: 900–1200 mg/day; CrCl 30–59: 600–900 mg/day; CrCl 15–29: 300–600 mg/day in very divided doses; CrCl <15 or dialysis: avoid if possible or single post-dialysis dose. Check levels more frequently with declining CrCl.

How often should renal function be monitored in lithium-treated patients?

At initiation: baseline then at 6 months. Stable patients: eGFR, TSH, and calcium every 12 months. If eGFR declines >25% from baseline or any sign of polyuria/polydipsia: every 3 months. Lithium causes nephrogenic DI (>30% of patients) and tubulo-interstitial nephritis with long-term use. Consider lithium discontinuation if CrCl falls below 30–40 mL/min.
