NUTRIC score (with IL-6): 0 to 5 = low risk; 6 to 10 = high risk. Without IL-6: 0 to 4 = low risk; 5 to 9 = high risk. High NUTRIC patients achieve significantly lower mortality with increased caloric delivery (up to target). Low NUTRIC patients may not benefit from aggressive calorie provision early (permissive underfeeding acceptable in first 3 to 5 days).

What caloric targets are recommended for high NUTRIC ICU patients?

High NUTRIC (score >=6): target 80 to 100% of estimated caloric needs (25 to 30 kcal/kg/day) and 1.2 to 1.5 g/kg/day protein. Low NUTRIC (score <=5): 70% target acceptable in first week (permissive underfeeding). Indirect calorimetry is gold standard for caloric needs; predictive equations (Harris-Benedict, Penn State) are acceptable alternatives.
