Why is the pediatric Parkland formula 3 mL/kg instead of 4 mL/kg?

Children require additional maintenance fluids on top of resuscitation fluids (calculated by Holliday-Segar). Using 3 mL/kg/% TBSA for resuscitation plus maintenance avoids over-resuscitation, which can cause abdominal compartment syndrome.

How are Parkland fluids administered?

Half the calculated 24-hour volume is given in the first 8 hours (calculated from time of burn, not hospital arrival). The remaining half is given over the next 16 hours. Time to hospital admission must be accounted for.

What endpoint guides pediatric burn fluid resuscitation?

Urine output is the primary endpoint: target 0.5-1 mL/kg/hr in children (higher than adult target of 0.5 mL/kg/hr) to ensure adequate renal perfusion. Adjust fluid rate based on actual urine output.
