Estimated Plasma Volume (EPV) = 0.065 x weight (kg) x (1 minus hematocrit). Standard exchange: 1x EPV per session removes 60 to 70% of plasma constituents. 1.5x EPV: removes 75 to 80%. TTP: daily exchanges until remission (platelet count >150x10^9/L x2 consecutive days). Anti-GBM: daily or every other day x 14 days with immunosuppression.

What replacement fluid is used in therapeutic plasma exchange?

FFP replacement: TTP (mandatory - replaces ADAMTS13 and vWF cleaving protease), anti-GBM (if bleeding risk high). 5% albumin replacement: MG crisis, GBS, CIDP, NMO, multiple sclerosis relapse. Avoid albumin in TTP (does not supply ADAMTS13). Ratio 1:1 replacement to exchange volume. Albumin + 30% FFP at end reduces bleeding risk from iatrogenic coagulopathy.
