ECOG PS 2 is highly heterogeneous. Disease-related PS 2 (from tumor burden) can improve with effective therapy; comorbidity-related PS 2 indicates impaired tolerance. Geriatric assessment tools (G8, VES-13) or CGA better inform decision-making than PS alone for PS 2 patients.

What survival benefit threshold justifies chemotherapy in PS 3 patients?

No evidence supports survival benefit of cytotoxic chemotherapy in ECOG PS 3–4 for most solid tumors. Exceptions include highly chemosensitive malignancies (SCLC, NHL, germ cell tumors) where PS-improving response is possible. Best supportive care is the evidence-based default for PS 4.
