SCORTEN = sum of: age >40 (1pt), malignancy present (1pt), tachycardia >120/min (1pt), initial BSA detached >10% (1pt), BUN >28 mg/dL (1pt), glucose >252 mg/dL (1pt), bicarbonate <20 mEq/L (1pt). SCORTEN 0–1: 3.2% mortality; 2: 12.1%; 3: 35.3%; 4: 58.3%; ≥5: 90%.

Where should SJS/TEN patients be managed?

Severe cases (BSA >10% or rapid progression) should be transferred to a burn unit or specialist dermatology center. Offending drug must be withdrawn immediately. Supportive care (wound dressings, fluid/electrolyte management, nutritional support, ophthalmology) is the cornerstone of management.
