This makes CRP and ESR unreliable for monitoring disease activity or infection in tocilizumab-treated patients. Clinical assessment, imaging, and other biomarkers (ferritin, fibrinogen) must substitute.

When is tocilizumab used for cytokine release syndrome?

CRS management (CAR-T/ICI-related): tocilizumab 8 mg/kg IV (max 800 mg) for Grade 2+ CRS (hypoxia, hypotension, or fever not responding to antipyretics).
