What defines severe UC by Truelove-Witts?

Severe UC: >= 6 bloody stools/day PLUS at least one systemic sign: pulse > 90 bpm, temperature > 37.8 C, haemoglobin 30 mm/h. These patients require hospital admission for IV steroids.

What is fulminant UC?

Fulminant or very severe UC: > 10 bloody stools/day, continuous bleeding, abdominal tenderness, fever, tachycardia, anaemia, ESR > 30. Risk of toxic megacolon. Requires urgent GI and surgical consultation.

How does Truelove-Witts guide inpatient management?

Severe UC: IV hydrocortisone 100 mg QID or methylprednisolone 60 mg IV once daily. If no response at Day 3 (Oxford criteria), rescue therapy with IV ciclosporin or infliximab, or colectomy evaluation.
