Stratifies VTE risk in surgical patients: Low (score 0–1) — early ambulation; Moderate (2) — pharmacologic prophylaxis (LMWH or UFH); High (3–4) — LMWH + mechanical; Highest (≥5) — LMWH + mechanical ± extended prophylaxis for 28 days.
