How is HAS-BLED score interpreted?

HAS-BLED 0–1: low bleeding risk (1.9/100 patient-years). Score 2: intermediate (3.74). Score ≥3: high bleeding risk (5.8+/100 patient-years). Score ≥3 does NOT automatically mean anticoagulation should be withheld—it flags modifiable risk factors for correction (uncontrolled HTN, labile INR, concurrent antiplatelet/NSAIDs, alcohol).

Should HAS-BLED be used to decide against anticoagulation in AF?

No. ESC guidelines explicitly state HAS-BLED should not be used to deny anticoagulation but to identify and address correctable risk factors. In AF, the stroke risk (CHA2DS2-VASc) almost always outweighs bleeding risk when score ≥2 (men) or ≥3 (women). DOACs have lower bleeding risk than warfarin and are preferred.
