The score averages four components scored 0–4 each: chest X-ray consolidation (0 = none, 4 = all quadrants), PaO2/FiO2 ratio (0 = ≥300, 4 = ≤100), PEEP level (0 = ≤5 cmH2O, 4 = ≥15 cmH2O), and respiratory compliance (0 = ≥80, 4 = ≤19 mL/cmH2O). Total score 0 = no injury, 0.1–2.5 = mild-moderate injury, >2.5 = severe (ARDS).

Is the Murray score still clinically relevant with the Berlin Definition?

The Murray score remains relevant for quantifying severity beyond the binary Berlin classification and for tracking changes in lung injury over time. It is also used in ECMO candidacy assessment — a Murray score >3 is one criterion used to consider referral for venovenous ECMO in refractory ARDS — and in research stratification where a continuous severity measure is preferred.
