What PUQE score indicates hyperemesis?

PUQE: mild ≤6, moderate 7-12, severe 13-15. Hyperemesis gravidarum is defined by weight loss >5% pre-pregnancy weight + ketonuria +/- electrolyte disturbance. PUQE >12 with dehydration warrants IV rehydration.

How is PUQE calculated?

Three items scored 1-5: hours of nausea per day (1=none, 5=>6h), number of vomiting episodes (1=none, 5=7+), number of retching episodes (1=none, 5=7+). Total score 3-15. Higher = more severe NVP.

What is first-line treatment for NVP?

Vitamin B6 (pyridoxine) 10-25 mg TID ± doxylamine is FDA-approved first line. Ginger 250 mg QID. Antihistamines (dimenhydrinate, promethazine) for persistent symptoms. Ondansetron for severe NVP (weigh risks in 1st trimester).
