Acceptable SBT methods include T-piece breathing, low-level CPAP (≤5 cmH2O), or minimal pressure support (≤8 cmH2O with PEEP ≤5). A 30-minute SBT is as effective as a 120-minute SBT for predicting extubation success. Failure criteria during SBT include: SpO2 35 or <8 breaths/min, hemodynamic instability, agitation, or increasing work of breathing.
