Ulcerative Colitis Mayo Score and Partial Mayo Index

Compute the four-part Mayo score for ulcerative colitis and its three-part partial version. Remission is 0 to 2, mild 3 to 5, moderate 6 to 10, and severe 11 to 12, with the endoscopic subscore reported separately.

Clinical Guidance

The Mayo score (also called the Disease Activity Index) combines four items — stool frequency, rectal bleeding, physician's global assessment, and endoscopic findings — each scored 0–3, for a total of 0–12. Scores of 0–2 indicate remission, 3–5 mild, 6–10 moderate, and 11–12 severe disease. A reduction of 3 points or more, or a score of 0 or 1, is commonly used to define clinical response or remission in trials.

The partial Mayo omits the endoscopic sub-score and uses only the three clinical items (0–9), which makes it suitable for frequent non-invasive monitoring between endoscopies. Because the full and partial scores use different denominators, compare like with like when tracking a patient over time.
