APACHE II Score — ICU Mortality Prediction

Calculate the Acute Physiology and Chronic Health Evaluation II (APACHE II) score using 12 acute physiologic variables, age, and chronic health status to predict ICU and hospital mortality.

Clinical Guidance

APACHE III is an ICU severity-of-illness score built from the worst physiologic values in the first 24 hours of admission, so it cannot be computed from a single set of vitals. Enter the most deranged value recorded for each variable during that window.

The total (0–299) reflects acute physiology points across 17 variables, an age modifier, and chronic health/comorbidity flags. Higher scores indicate greater physiologic derangement and higher predicted hospital mortality. Use the score for ICU benchmarking and research stratification — not as the sole basis for individual treatment decisions.
