ASPECTS divides the MCA territory into 10 regions: M1–M6 (cortical), and caudate, lentiform nucleus, internal capsule, and insular cortex. Each region with early ischemic change subtracts 1 point from the starting score of 10.

What ASPECTS threshold is used for endovascular thrombectomy?

ASPECTS ≥6 is the traditional threshold for thrombectomy eligibility. ASPECTS 0–5 indicates large established infarct with higher hemorrhagic transformation risk. Recent trials (ASPECTS 3–5) show benefit in selected patients at expert centers.
