What variables comprise the CPIS score?

CPIS uses six variables: temperature, blood leukocyte count, tracheal secretions (amount and character), oxygenation (PaO2/FiO2 ratio), chest X-ray infiltrates, and microbiological culture results of tracheal secretions. Each variable is scored 0–2; a total CPIS ≥6 suggests VAP. The modified CPIS omits the culture component for rapid bedside assessment.

How should CPIS guide antibiotic therapy decisions?

A CPIS ≥6 supports initiating or continuing antibiotic therapy for VAP, while CPIS <6 at 72 hours with improvement may support antibiotic discontinuation in patients started empirically, potentially reducing antibiotic exposure and resistance. However, CPIS has moderate sensitivity and specificity; clinical judgment and bronchoscopic cultures remain essential in ambiguous cases.
