What is the recommended CPP target in severe traumatic brain injury?

Brain Trauma Foundation (BTF) 2016 guidelines recommend a CPP target of 60–70 mmHg for severe TBI patients (GCS ≤8). The LEVEL 1 recommendation supports avoiding CPP 70 mmHg may increase systemic complications without additional benefit. The CPP-targeted approach (maintaining MAP to achieve CPP goals despite elevated ICP) should be balanced against volume status, vasopressor use, and the risk of secondary pulmonary complications from aggressive MAP augmentation.

How does cerebrovascular autoregulation affect CPP targets?

In normal autoregulation, cerebral blood flow (CBF) remains constant over a CPP range of ~50–150 mmHg. After severe TBI, autoregulation is impaired in many patients. Pressure-reactivity monitoring (correlation between ICP and MAP oscillations) can identify the optimal CPP (CPPopt) for individual patients — the CPP at which autoregulation is most active and outcomes are best. Personalized CPP targeting based on autoregulation indices is an emerging approach in neuro-ICU management.
