Nulliparity, engaged breech head, low amniotic fluid (AFI 3.8 kg, and frank breech (legs extended) reduce ECV success. Non-anterior placenta and posterior position of fetal back increase success.

What is the optimal timing for ECV?

Standard ECV at 36-37 weeks. ECV at 34-36 weeks (early ECV) has higher success rate but more spontaneous reversions. ECV in labor (ruptured membranes, early labor) has limited evidence but may be attempted in certain circumstances.
