What are the five factors in the FFS?

Proteinuria >1g/day (+1), creatinine >1.58mg/dL (+1), cardiomyopathy (+1), GI involvement (+1), and age >65 (+1). Each factor adds 1 point. Original 1996 FFS included ear, nose, throat involvement as a protective factor (−1 for EGPA).

What FFS score justifies cyclophosphamide?

FFS ≥1 predicts 5-year mortality >25% and justifies adding cyclophosphamide to corticosteroids. FFS 0 (low-risk) may be managed with steroids alone in PAN and EGPA.

Has the FFS been revised?

Yes. The 2011 revised FFS removed ear/nose/throat as protective factor for EGPA and updated cutoffs. Age >65 was added as a factor, and GI tract involvement was redefined.
