HAQ-DI >0.5 at 1 year predicts significant work disability at 5 years. HAQ-DI >1.0 is associated with increased cardiovascular mortality risk. A clinically meaningful difference is 0.22 points.

How is HAQ-DI calculated?

HAQ-DI = average of 8 category scores (dressing, arising, eating, walking, hygiene, reach, grip, activities). Each category uses the worst item in that category. Aid/device use elevates minimum score to 2 in that category.

Can HAQ-DI improve with RA treatment?

Yes. Effective biologic therapy (TNFi, IL-6Ri, JAKi) reduces HAQ-DI by 0.3-0.6 units in responders. Long-standing disability from joint damage may not fully reverse even with inflammation control.
