High-titer anti-U1-snRNP (anti-U1-RNP) is the hallmark and required for MCTD diagnosis. ANA is positive (speckled pattern). Anti-dsDNA and anti-Sm are typically absent, distinguishing MCTD from SLE.

What are the major features of MCTD?

Raynaud's phenomenon (>90%), swollen hands/puffy fingers (75%), inflammatory arthritis (75%), inflammatory myopathy (50-70%), pulmonary hypertension (25%). Renal disease is less common than in SLE.

What is the long-term prognosis of MCTD?

10-year survival is 80-90%. Major causes of death are pulmonary hypertension (the most serious complication, PAH in 25%) and infection. MCTD may evolve toward SSc or SLE dominant phenotype over decades.
