Cardiogenic: high PCWP, low CO, high SVR. Septic: normal/low PCWP, high CO (hyperdynamic), low SVR. Mixed patterns occur in sepsis-induced cardiomyopathy (high PCWP, low CO, low SVR).

What hemodynamic pattern suggests RV failure?

RV failure shows elevated CVP/RAP (above 15 mmHg), low or normal PCWP, low CO, and elevated PVR. The CVP/PCWP ratio above 0.8 and RAP/PCWP ratio above 0.86 are specific for RV-dominant shock.
