What is the Mehran CIN risk score?

Mehran score includes: hypotension (5 pts), IABP (5 pts), CHF (5 pts), age > 75 (4 pts), anaemia (3 pts), diabetes (3 pts), contrast volume (1 pt per 100 mL), eGFR 40-60 (2 pts), eGFR 20-40 (4 pts), eGFR = 16 = 57% CIN risk.

How can CIN be prevented?

Isotonic saline 1-1.5 mL/kg/h for 3-12 h pre-procedure and 6-12 h post-procedure. Minimise contrast volume. Use iso-osmolar contrast (iodixanol). Hold nephrotoxic drugs 24-48 h prior.

At what eGFR is contrast most dangerous?

Highest risk at eGFR < 30 mL/min/1.73 m2. Risk-benefit must be carefully weighed. eGFR < 15 or dialysis: contrast can be used but AKI definition changes; ensure pre-hydration.
