What effluent dose is recommended for CRRT?

KDIGO 2012 guidelines recommend a delivered effluent dose of 25-35 mL/kg/h. Higher doses (> 35 mL/kg/h) have not been shown to improve outcomes in large RCTs (ATN, RENAL trials).

How is CRRT effluent dose calculated?
