Dexmedetomidine (alpha-2 agonist) produces co-operative, arousable sedation (patients respond to voice, protect airway). MENDS2 and SPICE III trials: dex reduces delirium vs lorazepam but not vs propofol. Reduces morphine requirements, facilitates earlier extubation. Main limitations: bradycardia (20 to 35%), hypotension (30%), cost vs propofol.

Can dexmedetomidine be used for alcohol withdrawal in the ICU?

Dexmedetomidine is an effective adjunct (not monotherapy) for severe alcohol withdrawal syndrome and AWS-associated delirium. Reduces benzodiazepine requirements by 50 to 60% in CIWA-Ar-directed protocols. Does not prevent seizures (benzodiazepines required for seizure prophylaxis). Phenobarbital + dexmedetomidine combination effective for refractory AWS.
