When is IV iron indicated in CKD?

IV iron is indicated when ferritin < 200 ng/mL or TSAT < 20% in CKD patients. A therapeutic trial is reasonable when ferritin 200-500 ng/mL and TSAT < 30% in haemodialysis patients on ESAs.

What IV iron formulations are available?

Iron sucrose (Venofer): 100-200 mg per dialysis session. Ferric carboxymaltose (Injectafer): 750 mg x 2 doses. Low molecular weight iron dextran: larger single doses possible.

What are the target iron stores in dialysis patients?

KDIGO 2012: ferritin 200-500 ng/mL, TSAT 20-50%. Avoid ferritin > 500 ng/mL without clear hyporesponsiveness, due to risk of iron toxicity and infection.
