What does Lille >= 0.45 mean clinically?

Lille >= 0.45 at Day 7 predicts 6-month mortality > 75%. Corticosteroids should be discontinued - continuing does not improve survival and increases infection risk. Consider liver transplantation evaluation.

Can Lille score be calculated earlier than Day 7?

Lille was validated at Day 7. An early change in bilirubin (ECBL: bilirubin rise Day 3-4) has also been proposed as an early stopping rule but has less evidence than the 7-day Lille score.
