What are the MEWS parameters and scoring ranges?

MEWS scores five variables on a 0–3 scale: Systolic BP (0 = 101–199; 1 = 81–100 or 200+; 2 = 71–80; 3 = ≤70). Heart rate (0 = 51–100; 1 = 41–50 or 101–110; 2 = 111–129; 3 = ≤40 or ≥130). Respiratory rate (0 = 9–14; 1 = 15–20; 2 = 21–29; 3 = ≤8 or ≥30). Temperature (0 = 35–38.4°C; 1 = 34–34.9°C or 38.5–38.9°C; 2 = 32–33.9°C or 39+; 3 = ≤31.9°C). AVPU (0 = Alert; 1 = Voice; 2 = Pain; 3 = Unresponsive). Total 0–14; score ≥5 requires immediate medical review.

Is MEWS validated for detecting sepsis?

MEWS has moderate sensitivity for detecting sepsis (approximately 70–80%) but lower specificity than dedicated sepsis screening tools like qSOFA or NEWS2. MEWS ≥4–5 is associated with sepsis diagnosis in several studies. However, the Royal College of Physicians NHS recommends NEWS2 over MEWS for sepsis screening due to its superior validation and inclusion of SpO2 and supplemental oxygen as parameters.
