How does NEWS2 differ from NEWS and when is each used?

NEWS2 (2017 update) added a second SpO2 scale (Scale 2) for patients with hypercapnic respiratory failure who target SpO2 88–92%, preventing alarm fatigue from artificially low saturations in these patients. Scale 1 is used for all other patients (target SpO2 ≥95%). NEWS2 is mandated by NHS England for all adult inpatients and is recommended for prehospital use. Original NEWS remains in use in some international settings.

What NEWS2 thresholds trigger escalation protocols?

NEWS2 response thresholds: Score 0 = routine monitoring; Score 1–4 = increase monitoring frequency; Score 3 in any single parameter = clinical review by registered nurse; Score 5–6 = urgent clinical review by registrar or senior; Score 7+ = emergency response (consider ICU/HDU transfer). A NEWS2 score ≥7 has high sensitivity (~76%) for predicting cardiac arrest, unanticipated ICU admission, or 30-day mortality within 24 hours.
