BTF guidelines recommend CPP 60–70mmHg for severe TBI. CPP 70mmHg may increase ARDS risk from aggressive vasopressor use. Individualized CPP targets using cerebrovascular reactivity (PRx) are increasingly used at expert centers.

How is MAP managed to optimize CPP?

If ICP is elevated, MAP is raised using vasopressors (norepinephrine first-line) to maintain CPP ≥60mmHg. Simultaneously, ICP-reducing interventions (sedation, osmotherapy, CSF drainage) are used to lower the ICP component of the CPP equation.
