What are the current ASPEN 2022 caloric and protein targets for ICU patients?

ASPEN 2022 guidelines recommend: Calories — 25–30 kcal/kg/day (actual body weight for BMI 50). Protein — 1.2–2.0 g/kg/day for most ICU patients; 2.0–2.5 g/kg IBW for critically ill obese patients; ≥2.5 g/kg/day for CRRT patients to compensate for dialysis-related losses. Trophic feeding (10–20 kcal/h) is recommended for the first 24–48 hours of critical illness.

When should parenteral nutrition be initiated in ICU patients?

ASPEN guidelines recommend enteral nutrition (EN) as the first-line route when the GI tract is functional. Parenteral nutrition (PN) should be initiated only when EN is contraindicated or insufficient: high-output proximal fistula (>500 mL/day), intestinal obstruction, mesenteric ischemia, abdominal compartment syndrome, or intolerance to EN after adequate trials. Early PN (within 48h) in ICU patients who can receive some EN is not recommended; supplemental PN after 7 days of inadequate EN is an acceptable strategy per ESPEN.
